LIABILITY RELEASE. By submitting this Official Entry Form, I hereby agree to be bound by and comply with all CPAA tournament rules and regulations. I expressly assume all risks associated with the tournament and I hereby release Central Pro-Am Association,, its officers, employees, sponsors, and hosts from any and all damages, claims, demands, costs of expenses relating to injury of any persons or damage to any property which I may sustain or which I may cause by reason of participating in or connection with this tournament.
